Equipment Purchase Form

1.   DO NOT submit this form unless notified by Shonda to do so.
2.   When you receive your email from Shonda, complete the form below & select submit.  You will be taken to the PayPal form to enter your credit card details.  Fill out that form & submit.
3.   Pick up your equipment at the Fairgrounds on August 26 – 27 at the UKI trial site.

 

Shonda will be notified that your fees have been paid. Note that the correct fee amount must be paid prior to picking up your equipment.
